Health Literacy in the Care-at-Home Setting

This 90-minute course introduces health literacy in the home care setting and explains why it is essential for safe, effective and patient-centered care. Learners will explore the definitions of personal and organizational health literacy, identify populations at risk for limited health literacy, recognize common red flags and examine social and environmental factors that affect understanding at home. The course also provides practical strategies, such as plain language, visuals, hands-on teaching, caregiver involvement and the Teach-Back method to improve patient comprehension, support adherence, promote independence and strengthen documentation and care planning.
